Corneal collagen cross-linking (CXL) has revolutionized the management of progressive keratoconus and iatrogenic keratectasia over the last 10 years. Epithelium-off CXL procedure, as described by the classic Dresden protocol, remains the ’’gold standard’’ in CXL treatment.
However, emerging CXL modalities, such as epithelium-on procedures, high-fluence CXL and combined treatments, promise substantial enhancement of CXL, eliminating current application disadvantages and increasing its efficacy.

Moreover, the spectrum of indications is considerably expanding, including pellucid marginal degeneration and refractory infectious keratitis in conditions being successfully  managed by CXL.
Scleral CXL represents the challenge of the future, introducing novel application targets with myopia treatment and innovative glaucoma management.

This talk emphasizes on recent advances in CXL technology with relevant clinical impact for the practicing physician, highlighting novel future perspectives and challenges.
